چکیده
1. The authors have now clarified which fluids they mean: 0.45% saline in 5% dextrose AND 0.9% saline in 5% dextrose, which are referred to as hypotonic and isotonic respecitvely. Whilst I understand that this is common parlance in paediatric fluid therapy, it is misleading without some qualification. Both these fluids are HYPERTONIC, but are regarded as EFFECTIVELY hypotonic or isotonic (respectively) because the dextrose is metabolised after infusion. This should be made clear in order not to confuse non-paediatricians unfamiliar with this usage.

Isotonic versus hypotonic maintenance IV fluids in hospitalized children: a meta-analysis.
OBJECTIVE To assess evidence from randomized controlled trials (RCTs) on the safety of isotonic versus hypotonic intravenous (IV) maintenance fluids in hospitalized children. METHODS We searched PubMed, Embase, Cochrane Library, and clinicaltrials.gov (up to April 11, 2013) for RCTs that compared isotonic to hypotonic maintenance IV fluid therapy in hospitalized children. Relative risk (RR), ...

OBJECTIVE To determine whether the use of hypotonic vs isotonic maintenance fluids confers an increased risk of hyponatremia in hospitalized children. STUDY DESIGN A search of MEDLINE (1946 to January 2013), the Cochrane Central Registry (1991 to December 2012), Cumulative Index for Nursing and Allied Health Literature (1990 to December 2012), and Pediatric Academic Societies (2000-2012) abst...
متن کاملA randomized controlled trial of isotonic versus hypotonic maintenance intravenous fluids in hospitalized children
BACKGROUND Isotonic saline has been proposed as a safer alternative to traditional hypotonic solutions for intravenous (IV) maintenance fluids to prevent hyponatremia. However, the optimal tonicity of maintenance intravenous fluids in hospitalized children has not been determined. The objective of this study was to estimate and compare the rates of change in serum sodium ([Na]) for patients adm...

OBJECTIVE To compare the safety and efficacy of isotonic versus hypotonic maintenance fluid in children. DESIGN Randomized controlled trial. SETTING Tertiary-level teaching hospital. PARTICIPANTS 60 children (age 0.5 to 12 years) who were admitted and anticipated to receive intravenous fluid for the next 48 hours. INTERVENTION Hypotonic fluid (Standard maintenance volume as 0.18% NaCl i...
